Make your reasoning obvious. If you don't actually show know why you're quitting, you won't be able to stick to it. Decide why it's time to move on.
Publicise your plan. If you tell all of your friends and family that you're quitting, it's an incentive to follow through, and they will surely assist your efforts in any way that they can.

Start a fun new hobby. If your addiction was something time consuming like gaming, you'll need a new hobby to fill in this new time slot. Be sure to mix around what you're doing, as to not fall into the same fate with something new!

^That seems like something that works on a personal basis, but it's harder for some people than others. For some things, skipping for 2 days is natural, and they just come right back to it.
